Consent to contact you

When you submit a form and check the box, you agree that Simply Health Corp and the licensed agent named on the form may contact you by phone, text and email about the coverage you asked about, including by automated technology or a prerecorded or AI voice. Consent isn't required to buy. You can withdraw it any time by replying STOP or asking your agent.

Your consent covers only the agency and agent named. It is not shared with other companies.

Third-party marketing organization (TPMO) disclosure

Simply Health Corp is an independent health insurance agency. Each agent is individually licensed in the states shown on their profile. Not all products are available in every state.

Simply Health Corp and its agents are paid by the insurance companies whose plans we offer. This does not change the premium you pay. Asking for a quote never obligates you to enroll.

We are not affiliated with HealthCare.gov, any state marketplace, or any government agency. Plan availability, benefits, prices and savings depend on where you live and your household. Enrollment is limited to certain times of year unless you qualify for a Special Enrollment Period.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov, 1-800-MEDICARE, or your local State Health Insurance Program (SHIP) to get information on all of your options. Not connected with or endorsed by the U.S. government or the federal Medicare program.

Scope of Appointment (Medicare)

Before an appointment about Medicare Advantage or drug plans, we'll ask you to complete a Scope of Appointment form, as Medicare requires. Signing it doesn't obligate you to enroll.
