Citizenship or immigration status

• U.S. passport or birth certificateFor citizens.
• Naturalization certificateIf you became a citizen.
• Green card (I-551)For permanent residents.
• Other current immigration documentFor example, an I-94 or a work permit (I-766).

For a life event

• Proof of lost coverageA letter from your employer or Medicaid with the end date.
